The 3 Primary Pillars to Scale a Home Care Business


Over the years, I’ve worked with home care agencies at just about every stage of growth.

I’ve seen owners struggling to get their first few clients….agencies trying to break through the $1 million mark….and owners doing several million dollars in revenue who are frustrated because they’ve hit another ceiling.

And while the problems may look different at each stage, I’ve found that most growth challenges eventually come back to three things.

I call them the 3 Primary Pillars of Scaling a Home Care Business:

1. Consistently generating new clients

2. Finding and keeping enough quality caregivers to serve them

3. Building a management system that allows the company to grow without everything depending on the owner

You can have great technology, a beautiful website, strong culture, great training and dozens of other things….

But if one of these three pillars is weak, eventually it becomes the bottleneck that stops your growth.

And here’s what’s interesting….

The bottleneck changes as you grow…..Let’s look at each one.

PILLAR #1: Consistently Generating New Clients

Obviously, you can’t grow without clients.

But there’s an important distinction between getting clients and having a predictable client acquisition system.

Most home care agencies can get clients…..

….The owner knows a few people.

….A hospital sends a referral here or there.

…..Past clients tell friends and family..

…..A client finds you on Google.

…..You develop a relationship with a skilled nursing facility or elder law attorney.

The phone rings….That’s great….

But that’s not necessarily always predictable.   Look at your numbers…it tells the story.

Do you keep hitting a ceiling? Or hit a number that you’re excited about ..then the numbers drop again

And it’s difficult to scale something that isn’t predictable.

If I asked you today:

“How many qualified leads will your agency generate next month?”

Could you answer with reasonable confidence?

What about:

“Where will those leads come from?”

Or:

“How many referrals should your salesperson generate based on the activities they’re doing this week?”

That’s where many agencies struggle.

The owner may have personally built some incredible referral relationships….but those relationships live inside the owner’s head.

Then they hire a salesperson and tell them to “go build relationships.”

….Six months later, they’re wondering why the salesperson isn’t producing.

To scale, you need to turn sales from something that someone does into a system the company owns.

That means knowing:

  • Who are our ideal clients?
  • What referral sources are we most likely to find them?
  • What makes us different?
  • What problems do we solve for those referral sources?
  • What messaging will actually land with the referral sources?
  • What activities actually generate opportunities?
  • How often should we be following up?
  • What should our salesperson be doing every day?
  • What KPIs tell us whether they’re on track?

….And then what happens once a lead comes in?   Generating leads is only half the equation.

If you’re generating 30 inquiries and converting 15% to 20%  of them, you may not need more leads….

You may need to fix conversions.

When you understand the numbers, growth becomes much less mysterious.

And eventually, your goal should be simple:

The agency can consistently generate new clients without the owner personally having to generate every referral.

That’s Pillar #1.

PILLAR #2: Finding AND Keeping Quality Caregivers

This is where home care gets interesting.

Because you can become too successful at Pillar #1.

You generate leads….The phone starts ringing….You conduct assessments….Families say yes….

And then someone in the office says:  “We don’t have anyone to staff it.”

Marketing generates demand that sometimes the agency can’t fulfill.

….The worst response is to simply recruit more caregivers.   Of course recruiting matters….

But just like sales, random activity isn’t a system.

Posting another job on Indeed isn’t a recruiting strategy.

A scalable recruiting system needs to answer some basic questions:

  • How many applicants do we need each week?
  • Where should they come from?
  • How quickly are we contacting them?
  • What percentage schedule interviews?
  • What percentage show up?
  • What percentage get hired?
  • How many complete onboarding?
  • How many actually work their first shift?

….And perhaps most importantly….

How many are still working 30, 60 and 90 days later?

Because hiring 30 caregivers doesn’t accomplish much if 20 disappear.

That’s why I intentionally call this pillar:

Finding AND keeping quality caregivers.

Retention starts before the caregiver is hired.

  • Are our recruiting ads effective or are they just like everyone else’s?
  • Did we hire the right person?
  • Did we understand what hours they wanted?
  • Did we understand how far they were willing to travel?
  • Did we match them appropriately?
  • Did we get them excited about what they are becoming a part of?
  • Did we tap into emotion in the process?
  • Did we prepare them for the client?
  • Did someone check in after their first shift?
  • Do they feel appreciated?
  • Does their supervisor communicate with them?
  • Do we  have a growth path for them?
  • Do we recognize when a caregiver/client match is starting to go sideways before somebody quits?

Great agencies don’t simply recruit caregivers….They create a caregiver engine.

Applicants enter consistently…..The right people move through the hiring process quickly.

New hires get appropriately matched…..Good caregivers receive enough hours.

And the organization works intentionally to keep them.

When that happens, recruiting stops being an emergency every time sales brings in a new client.

PILLAR #3: Building a Management System That Eliminates Chaos

This is the pillar I think owners underestimate the most.

You can brute-force Pillars #1 and #2 for quite a while.

The owner sells…The owner recruits…The owner solves scheduling problems….The owner answers questions.

The owner talks to upset families….The owner approves everything.

And revenue grows.

From the outside, it looks like you’re building a successful company.

But sometimes what you’ve actually built is….A bigger job for yourself.

Eventually, the very thing that helped the company grow becomes the thing preventing it from growing further……You.

Not because you’re doing something wrong….But because there is only one of you.

This is where the third pillar becomes critical.

You need a management operating system.

Everyone should know:

  • What am I responsible for?
  • What outcomes do I own?
  • How is my performance measured?
  • What decisions can I make without asking the owner?
  • What numbers should I be looking at every week?
  • What happens when those numbers are off?
  • There should be clear accountability.
  • Clear KPIs.
  • Regular meetings with a purpose.
  • Defined roles.
  • Documented processes.
  • Decision-making authority.
  • And managers who actually lead, manage, and hold people accountable

One of my favorite questions to ask owners is:

“If your agency doubled in revenue over the next 24 months, what would break first?”

It’s usually not difficult for them to answer.

…Scheduling.

….Recruiting.

….Their administrator.

….Sales management.

…..Communication.

…..The owner.

Whatever your answer is….

That’s probably where your next management system needs to be built.

Because the goal isn’t simply to build a larger home care agency.

The goal is to build an organization capable of handling a larger home care agency.

….There’s a big difference.

The Pillars Have to Grow Together

Here’s where this becomes powerful.

Imagine these three pillars holding up your company.

You dramatically improve sales….But recruiting can’t keep up…..Growth stops.

You recruit 50 amazing caregivers…..But don’t have enough clients.

They leave.

You solve sales and recruiting…….But every decision still goes through the owner.

Eventually, the owner becomes the bottleneck.

That’s why scaling isn’t about fixing one department.

It’s about identifying which pillar is currently limiting the company….strengthening it….and then finding the next constraint.

At $1 million, your bottleneck might be consistent referrals.

At $2 million, it might be caregiver recruiting.

At $4 million, it might be leadership.

At $7 million, it might be your organizational structure.

At $10M+ it might be leadership development

There’s no magic revenue number where each problem appears….But the principle remains the same.

As your company grows, your systems have to grow with it.And ultimately, I believe the goal is to build a home care company where:

Clients are generated predictably….Quality caregivers are recruited and retained consistently.

And a strong management team runs the day-to-day business without everything depending on you.

….That’s when you’re no longer just growing a home care agency….You’re building a scalable company.

So here’s the question I’d challenge you to think about this week:

Of the three pillars….Sales, Caregivers or Management….which one is currently preventing your agency from reaching its next level?
